Acute Inpatient Rehabilitation Unit

º¬Ðß²ÝÑо¿Ëù Our Acute Inpatient Rehab Unit

Patients admitted to our acute inpatient rehab facility (also called an IRF) participate in an intensive rehab program for approximately 3 hours a day, 5-6 days a week.

Stays are typically 10-14 days, and the rehab team works closely with patients to improve their functional recovery and facilitate a safe return to the community. 

Our rehab team consists of a board-certified rehabilitation physician, a resident physician training in Physical Medicine and Rehabilitation, a physician’s assistant, rehabilitation nurses, physical therapists, occupational therapists, speech language pathologists, a case manager, a social worker, and a rehabilitation coordinator. The rehab team meets weekly during the patient’s stay to discuss progress, identify, address discharge barriers and problem solve collaboratively with the patient and family members to create a safe discharge plan. 

Patients who have recently experienced significant illness or injury that impacts their ability to participate in previous life roles, may need intensive, hospital-based therapy before returning home and resuming their life roles and responsibilities.

These are some of the qualifying diagnoses including, but not limited to:

• Stroke
• Amputation
• Neurologic/Neuromuscular Disorders including: Multiple Sclerosis, Parkinson’s, Guillain-Barre Syndrome, Muscular Dystrophies, and Neuropathies
• Traumatic Brain Injury
• Spinal Cord Injury
• Orthopedic Conditions including: Joint Replacements, Fractures, and Arthritis
• Trauma including: Motor Vehicle Accidents or s/p fall with resulting hip fracture or multiple fractures/poly trauma
• Cardiac Conditions
• Respiratory Conditions

Criteria for Admission

• Medically stable and able to tolerate and actively participate in 3 hours of therapy a day
• Demonstrate need for: skilled rehab nursing care, close medical monitoring, and care plan management by a rehab physician
• Demonstrate the ability to make gains towards goal to return to community 

Patients may be referred to a Subacute Rehab (SAR) or a Skilled Nursing Facility (SNF) if their current care providers deem this a better fit based on diagnosis and functional recovery or if unable to tolerate intensive daily therapy. 

Discharge

A discharge plan is developed soon after arriving on the inpatient unit. The goal is for the patient to successfully return home or another community living option.  Our social work and case management team work closely with patients and their families to ensure a safe discharge to the most appropriate setting.
